Junior Volunteer Program - Middlesboro ARH

This program is for High school students aged 14-18 years old where they will get to volunteer in clinical and non-clinical settings.

The program is 7 weeks at a time and begins October 9th. 

All volunteers will receive an ARH polo shirt and name badge.

All volunteers will be asked to wear khaki pants with their shirt provided. 

 

Before Volunteering all of the following forms must be signed and on file: 

  • Letter of Reference from a High School Teacher 
  • Copy of Immunization Record 
  • Copy of School Physical 
  • TB LABs to be done for each student at Middlesboro ARH Lab
  • All Orientation forms read and signed by both student and parent:
    • COVID Education Form
    • Cell phone form
    • Middlesboro ARH Orientation Packet
    • Application for JR. Volunteer Position and signed job description.
